29 CFR 1926.1053(b)(6):Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement. On or about July 13, 2021 at the job site located at 175 Hickory Lane, Moreland Hills, Ohio 44022; The 8 foot self-supporting ladder was set up to gain access to the elevated area where the employees were drilling 2, 3 ? inch holes through the exterior of the new construction residential structure. The self-supporting ladder was set up and accessed on uneven ground which caused it to displace.
