29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy controls: (a) On or about July 8, 2021, at the worksite located at 1450 E. Bolivar Avenue, Saint Francis, WI, the employer did not ensure the established procedure clearly and specifically outlined the requirements for testing the equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy controls. Hazardous energy control procedures were established for the Effytec feeder system # 16, however, the procedures did not contain adequate information pertaining to, at minimum, the verification that hazardous energy was isolated. (b) On or about July 8, 2021, at the worksite located at 1450 E. Bolivar Avenue, Saint Francis, WI, the employer did not ensure the established procedure clearly and specifically outlined the requirements for testing the equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy controls. Hazardous energy control procedures were established for the Effytec feeder system # 17, however, the procedures did not contain adequate information pertaining to, at minimum, the verification that hazardous energy was isolated.
