29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about June 28, 2021, the employer's lockout procedure for the Double End Tenorer (DET) did not specify the coast down time needed to delay the entry of the authorized employee to the hogger blades. The blades took 6 to 7 minutes to stop motion once the electrical energy was disconnected and locked out. An employee suffered multiple injuries to the right hand from contact with the right hogger blade when attempting to remove a jammed door from the DET machine's first pass section.
