29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: On or about June 23, 2021, at the location of 5535 Wegman Rd., Valley City, OH 44280, line 12, die cut, adequate precautions were not taken to prevent the ignition of flammable vapors when cutting or welding was permitted. Carco paint (red and all others) and thinner, which are a Category 1 flammable liquids integrated into the equipment, were not removed prior to cutting metal with a Miller Spectrum 875 plasma cutter, exposing employees to fire and explosion hazards.
