29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about June 28, 2021, on the north area of the compound, employees utilize aluminum ladders at heights upwards of twenty-five (25) feet without performing inspections of the ladders before initial use to identify defects.
