29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On or about 5/27/2021, 1901 13th Avenue East, Tuscaloosa, AL 35404, in the stockroom area - employees using Work Assist Vehicle (WAVE) platform were exposed to fall hazards up to 17 feet to the concrete floor because the employer did not ensure that the WAVE safety gate was repaired prior to being operated.
