29 CFR 1910.134(d)(3)(i)(B)(1):The employer did not select a respirator for employee use that maintained the employee's exposure to the hazardous substance, when measured outside the respirator, at or below the maximum use concentration: a) On or about May 26, 2021, an employee exceeded the respirator maximum use concentration of 10 when using a 3M 6000 half face respirator for digging out dome enclosures. The employee was exposed to respirable dust of 57.5mg/m3 for an 8-hour day, which is 11.5 times the permissible exposure limit of 5mg/m3. Sampling was conducted for a period of 389 minutes. Zero exposure was assumed for the remaining 91 minutes.
