29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about May 5, 2021, located at Section E and F, Rotary Feeder, employees were exposed to amputation hazards when performing service and maintenance tasks on equipment such as, but not limited to, the dust collector system without developing, documenting and utilizing procedures for controlling hazardous energy.
