29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) SRTA 2 Station 130 Machine: On or about March 31, 2021 and at times prior thereto, the employer exposed employees to struck-by and crush-by hazards in that the employer failed to ensure employees were utilizing machine specific lockout tagout procedures prior to entering the machine cell to perform maintenance activities. Mando America Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1263453, citation number 1, item number 1 (a) and was affirmed as a final order on 11-30-2017, with respect to a workplace located at 4201 North Park Drive, Opelika, AL 36801.
