29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about March 29, 2021, employees were exposed to a struck by hazard while performing logging sawmill operations at M&T Hardwoods, LTD. Employees were operating a Volvo 2200 model 7DZXL06-1063 Fork Truck to move and stack tree trunks in storage areas in the main yard and the employer did not ensure that the operators were trained to operate the Fork Truck safely.
