29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 2/25/2021, the employer failed to ensure that lockout/tagout procedures were developed, documented and utilized for machinery including, but not limited to the Warner & Swasey Turret lathe #5. Employees were tasked with repairing the lathe without utilizing lockout/tagout procedures. Energy sources including, but not limited to electric, pneumatic, and mechanical.
