29 CFR 1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) JPAM Management & Consulting, Inc. d/b/a Avalon Garden Skilled Nursing & Rehabilitation Center located at 4359 Taft Avenue, St. Louis, Missouri: On or about January 28, 2021, the employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix). The employer required employees to wear respirators throughout their shifts while exposed to suspected or confirmed positive COVID-19 residents.
