29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Area below Startup Cyclone. On or about February 21, 2021, employees performed servicing and maintenance work including, but not limited to, clearing jams from the Startup Cyclone. The energy control (lockout/ tagout) procedures for the cyclone did not include specific procedures and techniques to prevent the unexpected release of stored energy and steam from the cyclone. The procedure lacked the following elements which included but were not limited to: (1) Specifically the identification of all energy sources and magnitudes of energy for each machine. (2) Specific identification of isolation points/locations for each energy source. The specific energy isolation points/locations that were not identified were associated with energy sources that included but were not limited to thermal energy (steam). (3) Specific sequential techniques for bringing the systems to a zero energy state (4) Specific techniques for verifying energy isolation (5) Specific methods/techniques to dissipate/disable stored energy sources such as steam (thermal energy)
