29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) In the facility workshop, on or about January 21, 2021 and at times prior thereto, the procedures to control potentially hazardous energy when Maintenance Technicians (MTs) and Maintenance Assemblers (MAs) connected and disconnected Maxwell MXW4 and MXW6 drill tools to the drill collar roll machines (DCRMs) were not utilized, exposing employees to caught-in and struck-by hazards.
