1910.147(d)(4)(i) Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. 29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools. On or about January 20, 2021, and at times prior thereto, nightshift sanitation employees would leave keys inserted into locks used to prevent unexpected energization of hazardous machinery, including but not limited to Bay 1 grinders, exposing employees to the hazard of being caught-in moving parts.
