29 CFR 1928.57(a)(6)(iii):Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment. except where the machine must be running to be properly serviced or maintained, in which case the employer shall instruct employees as to all steps and procedures which are necessary to safely service or maintain the equipment; a) Jobsite: On or about December 22, 2020 and at times prior thereto, the employee was exposed to a caught-in hazard from the moving machinery parts of the rollers that move the Jerusalem Artichoke along the processing line. The employee performed cleaning and unclogging of equipment on the operation deck of a harvesting machine without disconnecting the power source while machine was moving. The employer failed to provide instruction on procedures on how to disconnect equipment which resulted in bodily injury to the employee.
