29 CFR 1910.134(c)(3): The employer did not designate a program administrator who was qualified by appropriate training or experience to administer or oversee the respiratory protection program and to conduct the required evaluations of program effectiveness: a) Ayden Healthcare d.b.a. Waterville Healthcare LLC., located at 8885 Browning Drive in Waterville, Ohio 43566: On or about November 18, 2020, and at times thereafter, the employer did not designate a program administrator who was qualified to administer or oversee the respiratory protection program. The employer required employees to wear N95 respirators, including while providing care to suspected and confirmed positive COVID-19 residents.
