Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment was covered or guarded to eliminate the hazard. On or about 11/03/2020, Fred Weber Reinforced Concrete Products, Inc. had not ensured employees working on or near a Besser, Model A-48 BiDi, concrete pipe manufacturing machine were protected from falling on a pallet turner located in a floor opening on a rotating table.
