29 CFR 1910.269(d)(8)(i)(E)): De-energization of all systems and reapplying of energy control measures was not done in accordance with paragraph (d)(6) of this section "established procedures for the application of energy control (the lockout of tag out procedures) to continue the servicing or maintenance". a) Lake Oliver Dam Turbine No. 2: On or about 10/27/2020, the employer exposed employees to caught-in hazards, in that procedures were not followed when removing and reapplying lock out for adjustments to the penstock priming valve in order to prevent pressure differentials from occurring at the valve.
