29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Alameda Center for Nursing and Rehabilitation, located at 303 Elm Street, Perth Amboy, NJ: On or about 4/23/2020 and continuing thereafter, the employer did not establish a written respiratory protection program. Certified Nursing Assistants and Licensed Practical Nurses were required to wear N95 respirators when providing direct care to suspected or confirmed COVID-19 positive patients.
