29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a) On or about August 28, 2020, at Dock #8, Door #18; an employee utilized Forklift #11 (a Toyota power industrial truck) that did not have an operable horn, functioning rear lights, and an operable reverse audible alarm.
