29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a. 1st, 2nd, and 3rd Avenues at 1276 Hanover Street, Manchester, New Hampshire - On or about June 11, 2020, the employer did not ensure that an effective respiratory protection program was established with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1901.134(c)(1)(i)-(ix) such as but not limited to fit testing and medical evaluations. Employees were required to wear N95 filtering facepiece respirators when entering and cleaning patient rooms and providing care to patients who were suspected or confirmed positive for COVID-19.
