29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite-specific procedures was not established and implemented for required respirator use: a) Clearview Operating Co., LLC dba The Grand Rehabilitation and Nursing, Queens, 157-15 19th Avenue, Whitestone, New York: On or about April 4, 2020 ,and continuing thereafter, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures (including how medical evaluations and fit testing will be completed) for employees (including Housekeeping Porters) required by the employer to wear N95 respirators due to exposure to COVID-19 positive residents or persons under investigation (PUI) for COVID-19.
