29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems were rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level: a. On or about June 3, 2020, on top of the outside resin storage tanks, employees were exposed to a 9 foot 11 inch fall hazard, in that, the fall arrest system installed and/or utilized would not prevent the employees from contact with the lower level.
