29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 1910.134(c)(1)(i)-(ix) with worksite specific procedures was not established and implemented for required respirator use: a) Center for Health, Education, Medicine and Dentistry 1771 Madison Ave Lakewood, NJ: A site specific written respiratory protection program that included, but was not limited to training, storage, and maintenance of respirators was not established for employees. The employer provided N95 respirators to employees for use, on or about 4/27/2020 and after. NOTE: IN ADDITION TO ABATEMENT DOCUMENTATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
