29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) On or about April 17, 2020, Greystone Healthcare Management Corp. did not ensure that a written respiratory protection program was established for those employees required to wear filtering facepiece respirators when providing care and assistance with activities of daily living to residents who were suspected or confirmed to have COVID-19 in the 1200 and 1300 wings. Employees were required to wear respiratory protection including, but not limited to, 3M 8200, SH9550 N95, SH9550C N95, or SH9550A N95 filtering facepiece respirators.
