29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Hallmark Healthcare LLC, 51 Madison Ave., Madison, NJ The employer failed to implement a written respiratory protection program for employees, including nursing and housekeeping staff, required to wear N95 respirators to provide care for and/or clean rooms of patients under investigation or confirmed as Covid-19 positive. Violation occurred on or about 04/23/2020 and thereafter.
