29 CFR 1910.27(b)(2)(v): The employer must ensure that the rope descent system has proper rigging, including anchorages and tiebacks, with particular emphasis on providing tiebacks when counterweights, cornice hooks, or similar non-permanent anchorages are used: a) Jobsite located at 2 Scott Way Building #2, Lester, PA.: The employer did not ensure that the rope descent system was provided a tieback when utilizing a counterweight system. Documented to occur on or about 23 FEB 2020.
