29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection: a) Hoppers for the dust collector systems: March 30, 2020 - in dust collector system hoppers, employees were exposed to a crush-by hazard while performing maintenance when the employer failed to ensure the "tags only" energy isolation procedure also included a "tags plus" isolating device to ensure the tags only procedure provided employee protection equivalent to a lock out and tag procedure.
