29 CFR 1910.28(b)(9)(i)(A): (A) Each fixed ladder installed before November 19, 2018 was not equipped with a personal fall arrest system, ladder safety system, cage, or well: 361 Main St., Greenfield, MA: On or about March 23, 2020 employees were allowed to access a fixed ladder installed prior to November 19, 2018 which had no fall arrest system, ladder safety system, cage or well installed on the fixed ladder. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
