On or about March 12, 2020, at the building roof, the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site and equipment to be made by a steel erection competent person. Employees placed and connected metal roof decking and were exposed to falls greater than 6 feet to a lower level.
