29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Building 23, depalletizing robot cell; On or about January 9, 2020, the energy isolation (lockout) procedure was not used while maintenance operations were performed on the pusher table section of an automated depalletizing robot (Fanuc, model M410-iB, robot # 2). An employee injury occurred, which resulted in hospitalization.
