29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: a. On or about 12/12/2019, at the fabrication area, employees were exposed to the inadvertent start-up or release of energy during maintenance of equipment such as but not limited to the Shear Lines and conveyor tables, cutting, bending and coil machines, in that, the energy control procedures did not have steps for testing machines or equipment to determine that the isolation for the control of an energy source such as electrical, hydraulic and pneumatic was effective.
