29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Kentwood M609HD Moulder Machine: On November 27, 2019 and at times prior, the employer exposed employees performing set-up operations on the moulder machine to amputation hazards in that the employer failed to develop, document, and utilize energy control procedures specific to the set-up process.
