29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) At the jobsite - On or about November 23, 2019 employees were exposed to amputation hazards, in that employees were allowed to perform jam clearing operations on equipment such as, but not limited to, non-woven slitter machine, without utilizing energy control procedures.
