29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area: a. Extrusion Line 3 - An employee suffered an amputation of two fingers when the rollers of the extrusion machine were lowered onto his hand while he was manually feeding the plastic through the rollers and that employee was not safely position and removed from the work area.
