29 CFR 1926.20(b)(2): The employers safety and health training program did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers: a) Rt 2, Friendly, WV- On or about 11/11/2019, the employer did not ensure work zones were inspected by a competent person to verify they were set up in accordance with the MUTCD 2009 Edition, Section 6C.05 with the appropriate length of tapers forming a transition area in the southbound lane of WV Route 2 when redirection of motorists' normal path was required, exposing the employees to work zone struck-by hazards.
