29 CFR 1910.303(h)(5)(iii)(A): Entrances to all buildings, rooms, or enclosures containing exposed live parts or exposed conductors operating at over 600 volts, nominal, were not kept locked or while unlocked were not under the observation of a qualified person at all times: On or about November 3, 2019, on north exterior side of the facility where an employee was exposed to electrical hazards after unlocking a switchgear enclosure and testing 13,800 volt exposed phases inside the switchgear without a qualified person being present.
