Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically outline specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about October 15, 2019, employees perform maintenance and servicing on various equipment. Employees were exposed to struck by, caught in and amputation hazards in that: a) Sorter Top Area Access - The lockout/tagout procedure includes a photo of the MCC room, Sorter triple arms, sorter drive , but does not indicate how to find the lock out points. b) Sorter Top Area Access - The lockout/tagout procedure does not indicate how to lock out the electrical panels at the MCC room. c) Sorter Top Area Access - The lockout/tagout procedure does not indicate how to lock out the sorter triple arms to shut off the air valves. d) Sorter Top Area Access- step 8 indicates "Release store energy- in the area to be locked out(bleed hydraulic, air , gas, steam, etc.." this does not explain how to release each stored energy. e) Sorter Top Area Access- When restoring power the procedures does not indicate in which order to reenergized the equipment.
