OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to caught-in and/or amputation hazards: a) On or about September 30, 2019, in Pup-01, the employer did not ensure that the foot switch or pedal on the Cincinnati 90 ton-capacity Hydraulic press brake, P-95 was in good working condition to prevent unintended activation of the equipment.
