29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed. a. On and/or before October 1, 2019, the employer did not ensure that the portable Little Giant LT lightweight multi-use ladder system (Model: 22 LT; Part Number: 14322-01) was used as designed with the Tip & Glide wheels positioned to the top of the ladder when used in the straight ladder mode/configuration, thus exposing employees to a fall hazard. b. On and/or before October 1, 2019, the employer did not ensure that the portable Little Giant LT lightweight multi-use ladder system (Model: 22 LT; Part Number: 14322-01) was used as designed by fully extending the top outer ladder assembly first before the bottom section was extended in the straight ladder mode/configuration, thus exposing employees to a fall hazard.
