29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: A) On or about October 7, 2019, the employer did not ensure precautions were taken to prevent filling liquid propane gas containers while the container was on or near an ignition source. LPG cylinders were filled while housed inside or on top of PIVs, which were parked approximately five (5) feet away from the LPG dispensing station. B) On or about October 7, 2019, the employer did not ensure flammable vapor source was controlled by permitting fuel station valves to remain open when an attendant was not present.
