29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs to provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On September or about 10, 2019 located near Orion station area there was no frequent and regular inspection program being maintained in use of the job site, materials and equipment by a competent person to ensure section(s) of track with cantilevered attached catwalk load rigged by a rough terrain forklifts elevated forks and sling attached to the catwalks guardrails while employees were underneath the cantilevered catwalk torqueing bolts or when sling(s) are released during the process were maintained and utilized by components to ensure structural stability of the load at all times without exposing employees working under the load to potential crushing hazards by the overturning structural steel section of track with cantilevered attached catwalk.
