29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:( A) Diamond Products, Limited, located at 333 Prospect Street, Elyria, OH 44035: On or about July 15, 2019, the employer did not develop and document specific procedures to control hazardous energy of the equipment such as, but not limited to, the Dr. Fritsch Cold Press # 5 (Hydraulic Unit, Type KP 35, Number KP 604) located in the Cold Press Department. Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic before conducting activities such as, but not limited to, changing dies. Employees were exposed to caught-in-between hazards that could result in injuries while all hazardous energy sources were not controlled prior to performing the tasks.
