29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about August 28, 2019, and at times prior thereto, the employer installed a makeshift power connection by splicing together vehicle jumper cables and a flexible cord and attaching to the outdoor air conditioning unit electrical bus bar. The assembly was used to power portable hand tools, exposing employees to an electrical shock hazard.
