29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: The employer is failing to protect employees from a struck by hazard when operating forklifts, powered industrial truck, at Pomp's Tire Service. This has most recently documented on or about August 30, 2019 at Pomp's Tire Service at 4101 West O Street, Lincoln, NE 68528. Employees operated a powered industrial truck used with a modified fork on a daily basis. One fork of the Kamutsu forklift has been cut back or modified from its original length, as compared to the length of the opposite fork.
