29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified per 29 CFR 1910.178(l): (a) Thirteenth Floor Entertainment Group, LLC DBA 13th Floor Haunted House at 3400 E 52nd Ave Denver, CO 80216: On and before August 26, 2019, the employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified per 29 CFR 1910.178(l). Employees operated a Datsun 2700 sit-down forklift without adequate training. Employees were exposed to struck-by hazards.
