29 CFR 1910.119(d)(3)(i)(B): The employer shall complete a compilation of written process safety information including information pertaining to the equipment in the process including piping and instrument diagrams (P&ID's): At a facility located at 191 W. 28th Avenue, Oshkosh, WI 54902; the employer did not complete a compilation of written process safety information. The P&ID for the anhydrous ammonia system was inaccurate due to: a) P&ID AC04 (Rev. 02) represented a connection to a portion of equipment near compressor AC-100, which had been partially disconnected from the main system. The P&ID inaccurately represented a connections that had physically been disconnected downstream of a tee junction between valves GV-128, GV-132 and GV-160 and upstream of PRV-300.
