29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. a) On July 22, 2019 the employer did not initiate or maintain a safety and health program that provides for frequent and regular inspection of the jobsite by a competent person, in that employees were exposed to struck by hazards while tapping the concrete formwork with a hammer next to an excavation wall without cave-in protection b) On July 22, 2019 the employer did not initiate or maintain a safety and health program that provides for frequent and regular inspection of the jobsite by a competent person, in that employees were exposed to fall hazards while walking on top of the nine foot high formwork to direct the flow of concrete.
