29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: A) Carbo Forge, Inc., located at 150 State Route 523, Fremont, OH 43420: On or about June 17, 2019, the employer did not ensure that the energy control procedure for the 530 Ton Hydraulic Shear (Advanced Machine Design Company, Model # 530-2230; Serial # 9102), located in the Shear Building, identified specific procedural steps how to control all hazardous energy sources such as, but not limited to, the gravitational energy of the shear. Employees did not control the gravitational energy of the shear before changing blades and were exposed to struck-by and pinch point hazards that could result in in amputations.
